The following forms are for all coaches, volunteers and Staff of St. Andrew

* Voluntary Certification for Child Care Services
Please download the forms listed above. They need to be completed and delivered to the CYO office. St. Andrew CYO will submit the clearance forms on your behalf to the appropriate state agencies.
CYO Volunteer Forms
Protecting God’s Children
Volunteers and/or coaches who have regular contact with children are required to complete parts one and two of the Safe Environment training within 90 days of beginning their volunteer work.
Part I: Protecting God’s Children (instructor-led), is offered regularly throughout the Archdiocese. The training provides basic facts about child sexual abuse and adults’ roles in prevention. Visit virtus.org to register for this class.
Part II: The Mandatory Reporting of Suspected Child Abuse (online), is available online at the training institute through the Archdiocese of Philadelphia. This training provides an overview of Archdiocesan reporting procedures for suspected child abuse and PA child welfare laws. The training takes approximately 45 minutes to complete.
